Eczema — atopic dermatitis in its commonest form — is a chronic inflammatory skin condition marked by dryness, intense itching, redness, oozing and, over time, thickened leathery patches where the skin has been scratched for years.

The defining feature is not the rash. It is the itch-scratch cycle. Scratching damages the skin barrier, the damaged barrier lets in irritants, the irritants inflame the skin further, and the itch worsens. Most patients describe the same trajectory: a mild childhood patch, a steroid cream that works, then a rash that spreads a little wider each time it returns.

Breaking that cycle needs work at two levels at once — settling the skin from outside, and removing what keeps generating the inflammation from inside.

Ayurveda places eczema under Kushtha [the classical category of chronic skin disease], and within it, the description of Vicharchika reads almost as a modern clinical note.

Kandu [severe itching], shyava [darkened, dull discolouration], pidaka [small raised eruptions] and bahusrava [copious oozing or discharge]. That last one is what separates Vicharchika from the drier scaling of psoriasis.

Kapha accounts for the oozing, thickening and stubbornness; Pitta for the heat, redness and burning. Where the skin is very dry, cracked and the itch is worst at night, Vata has entered the picture and the treatment changes accordingly.

As with every form of Kushtha, the texts describe seven factors disturbed together — all three doshas, plus twak [skin], rakta [blood], mamsa [muscle] and ambu [the body’s water and lymph]. Blood is the crucial one. When rakta dhatu [the blood tissue] is carrying impurity, no amount of external application will hold a result for long.

Weak Agni [digestive fire] leaves food incompletely processed and Ama [metabolic toxins] accumulating.

Ama enters circulation and vitiates the blood, which then irritates the skin from beneath.

Skin loses its ability to hold moisture and defend itself. Every external irritant now lands harder.

On eczema in children. Paediatric eczema is common and it is treated differently. Strong purification is not appropriate for young children. Treatment centres instead on correcting digestion, careful diet adjustment for the child and, where relevant, the mother, mild internal medicines and external applications. Most childhood eczema improves markedly with this alone.

Some of these are classical; some are simply what we see in Bengaluru patients week after week. Both matter.

  • Viruddha Ahara [incompatible food combinations] — particularly milk taken with fish, sour fruit or salt
  • Daily curd, especially at night, and heavy fermented food
  • Excess seafood, black gram and sour foods
  • Harsh soaps, detergents and long hot showers, which strip the skin barrier
  • Hard water and frequent hand washing — a common trigger for hand eczema
  • Dust, synthetic fabric and pet dander in atopic individuals
  • Sustained stress and broken sleep; the itch reliably worsens in a difficult month

Our Ayurveda Treatment Protocol

The sequence matters as much as the medicines. Purification before digestion is corrected does more harm than good. Durations below are typical for adults — yours is set after assessment.

Constitution, dosha pattern, lesion type and distribution, digestive history, allergy and asthma history, and everything currently being applied to the skin.

Bitter and digestive herbs restore Agni and start clearing accumulated Ama. Alongside this, the diet triggers come out — this stage alone often reduces the itch noticeably.

Medicated ghee taken internally in a rising daily dose — commonly Mahatiktaka Ghrita — to draw the disturbance out of deeper tissue and toward the gut, where it can be removed.

Virechana [therapeutic purgation] where heat, redness and burning dominate. Vamana [therapeutic emesis] where the picture is heavily Kapha — thick, oozing, spreading lesions with a history of allergy or asthma.

Diet rebuilt in graded steps from thin gruel back to normal food. Rushing this is the single commonest reason a well-executed purification fails to hold.

Blood-purifying decoctions, Rasayana [rejuvenative] support, and a sustained external routine that keeps the skin barrier intact. Reviewed monthly.

Medicines, Therapies and Daily care

Internal formulations

Chosen by presentation — a dry, cracked eczema and a weeping one are not given the same medicines:

  • Aragwadhadi Kashayam and Manjishtadi Kashayam — decoctions acting on rakta dhatu
  • Patolakaturohinyadi Kashayam where itching and heat dominate
  • Panchatikta Ghrita Guggulu and Arogyavardhini Vati
  • Gandhaka Rasayana and Khadirarishta through the maintenance phase

External applications

Matched to the state of the skin, and changed as it improves:

  • Jatyadi Ghrita or Jatyadi Taila for raw, weeping or fissured lesions
  • Nalpamaradi Tailam and Eladi Tailam for itching and darkened patches
  • Panchavalkala decoction wash to settle oozing and secondary infection
  • Takradhara [medicated buttermilk poured over the forehead] where the itch is clearly stress-driven

Skin care that supports treatment

Short lukewarm showers, no soap on affected areas, a mild non-foaming cleanser, cotton next to the skin, and moisturising within three minutes of drying. These sound minor. They decide how well everything else works.

Diet

We remove the incompatible combinations first, then test the usual suspects — curd, sour foods, seafood, black gram — one at a time rather than banning everything at once. A restriction you cannot sustain for six months is not a treatment.
